Dear applicant,

Thank you for applying with us at Hooks Home Bound Rescue. You have been asked to fill in this worksheet so we could get to know you better. Please complete to the best of your knowledge. We thank you for applying and opening your heart and home.

You are almost done with your application process. We are glad you can join us and allow us to get to know you more. We ask for this information to ensure a proper home is set for all our animals.

After completion and acceptance of all documents, you become part of our extended family. We look at all our animals as part of our family and we hope you would consider the animals the same way.

Do you understand this Animal legally belongs to Hooks Home Bound Rescue and that Hooks Home Bound Rescue holds all legal rights to this Animal and may revoke your fostership/adoption at any time for any reason? (unless otherwise signed off)*

Do you understand that if you are to adopt this Animal that you are then liable for all yours and the Animals actions after the signing of the foster/adoption agreement?*

Do you understand that the rescue makes no guarantees or statements regarding the Animal's age, breed, health, or temperament?*

Do you understand after signing the foster/adoption agreement it is your responsibility to ensure this Animal is wearing its identifications tags from the township you live in, Hooks Home Bound Rescue tag, and another identification tag with your information on it at all times?*

Do you understand that when signing the agreement it is your responsibility for the care and welfare of the Animal you wish to foster/adopt even if you wish to return the Animal?*

I agree if Hooks Home Bound Rescue requests relinquishment of any animal I may be fostering that I will do so within 72 hours of the request!*
